Activity: Sign into your Google Account and start a practice Form like the one in the video clip. Your practice form should include the following:
1. A title
2. Directions for filling out the form
3. Last and first name "text" style questions.
4. A multiple choice question related to this workshop

Activity: Go back to your practice form and add two questions on any topic you wish. These questions should each be of a different type, and should not be a text or multiple choice question.
